1.  That my maiden name is ___________.

2.  That I got married to Shri. (name of the husband) on (date of marriage)  at ____________(place of marriage).

3.  After marriage, my name is __________ and all the records have the same name.

4.  I have not obtained the marriage certificate.

5.  I state that (maiden name) and the (present name) is the name of one and the same person and that is myself.

I am executing this declaration to be submitted to the concerned authorities for the change of name.

I hereby state that whatever is stated herein above are true to the best of my knowledge.

1.  The affidavit should be on Non-judicial stamp paper of Rs.20/- and on Rs.100/- if the applicant is resident of Maharashtra.

Please type this Affidavit after carefully reading the notes mentioned below the format.

I, __________ son/daughter/spouse of __________ aged ___, residing at __(address)___, do hereby solemnly affirm and declare as under:

That I, Shri/Smt. ————–S/W/D/o……………….aged……Occupation ………., R/o…………..

I am residing at the above-mentioned address and have not changed my address in the past three years.

OR

I am residing at the above-mentioned address for the past (period of time at the present address) and before that, I was staying at (the earlier address) for (period of time at the earlier address).

That I have changed my signature.

That my signature has changed/altered by passing of time.
